Asthma Testing

The CHADIS Asthma Questions include many tools to help our doctors assess and monitor your child’s asthma. When you arrive for your appointment your child will have special lung function tests to assist with their care if they are five years of age and older. If your child has persistent problems with their asthma (3 times per year or more), you will need to schedule an appointment every three months to help us monitor their condition. If your child has occasional symptoms (1-2 times per year), then they are required to have an asthma visit once per year outside of their well exam.

If your child has a chronic cough and/or wheezing here are the next steps you should complete prior to your appointment:

1. Asthma Assessment Tools

  • Go to chadis.comand create your child’s account if you have not already done so, then complete the following questionnaires:
  • (PACCI) :  The PACCI documents Pediatric Asthma Control & Communication Instrument helps your providers determine which medication may need to be prescribed.
  • Childhood Asthma Control Test (ACT) for Ages 4-11 which screens for possible problems in control of asthma symptoms.
  • Asthma History :  A tool that documents your child’s birth history, subsequent respiratory risk factors, and any family history of asthma and allergies.
  • Asthma Priorities : This brief, three-question tool collects information about the parent’s visit priorities and the hardest parts of caring for a child with asthma, which can be used to guide the visit.
  • Asthma Trigger Exposure Questionnaire :  This identifies any common triggers for allergies or asthma includingpollen, molds, animals, dust, foods, cockroaches, and smoke.
  • Adolescent Self Report : Asthma Control Test (ACT) for People 12 Years and Older: Screens for possible problems in the control of asthma symptoms in older children.

2. Asthma Treatment Plan (Asthma Action Plan)

  • Asthma Treatment Plan : Using these tools, a printed plan with color guided sections for well/worse/sick situations will be generated with specific medication instructions.  The treatment plan will be located in your child’s Care Portal and can be printed as needed by your family.
