Decatur Pediatric Group requires each patient to present proof of insurance and photo ID at every visit.
Co-payments are expected at the time of service.
We are requiring that all patients sign a new Financial Statement
prior to your visit to be in compliance with our financial commitment.

Insurance plans and contracts change frequently so be sure to contact our practice or your insurance plan to verify participation.
Our Hillandale location does not participate in any Medicaid nor Medicaid CMO plans.
Most insurance companies require the member to select a PCP (Primary Care Physician). If you would like to use a provider with Decatur Pediatric Group and you have already selected a PCP that is not a Decatur Pediatric Group provider, please contact your insurance carrier to have your PCP changed. If the PCP is not changed through your insurance carrier, the insurance carrier may not cover charges. For more a better understanding of your insurance company requirements, please contact your insurance carrier or your employer's human resource department.
